Accident Investigation - Incident Cause Analysis Method (ICAM)
The accident investigation course is delivered to meet NZQA’s criteria through using the ICAM model for investigation.
This course is delivered using our blended training model. The online module unit standard 17601 – Incident Investigation, is to be completed prior to attending your 1 day interactive in person training for ICAM methodology.

- Most accidents/incidents happen, due to organisations failing to learn or retain the lessons that should have been learned from previous accidents and incidents
- Often witnesses memories become impaired due to the loss of time span or the information may become tainted or impaired
- All workplace accidents/incidents should be investigated as soon as possible after the occurrence of either an accident or incidence
People credited with this unit standard are able to: explain why workplace accidents need to be reported and investigated; explain the concepts of multiple causation and root-cause analysis; detail an incident investigation process using a hypothetical incident; and produce an incident investigation report based on a documented incident.
